Part D is Medicare's prescription drug benefit, offered through private insurance plans approved by Medicare. If you're on Original Medicare (Parts A and B), you can add a standalone Prescription Drug Plan to your coverage. If you're enrolled in a Medicare Advantage plan, prescription drug coverage is typically built in.
Either way, the plan you choose determines which medications are covered, at what cost tier, and through which pharmacies. Formularies — the lists of covered drugs — vary from plan to plan, and they change every year. A plan that covered your medications well in 2024 may not be the best fit for 2025.

That said, late enrollment situations often have more options than people expect. If you had creditable coverage before enrolling — even if you didn't realize it qualified — the penalty may not apply at all. Annual Plan Reviews Every Fall During AEP
Part D plans are not set-and-forget coverage. Each fall, insurance carriers adjust their formularies, cost-sharing structures, and premiums for the following year. During the Annual Enrollment Period — October 15 through December 7 — anyone on Medicare can switch their Part D plan without penalty.
